Yes, I've observed this too (the first phenomenon) on a Dell Optiplex PC (using its built in sound) running Windows XP. It appears unless a small noise is made, the audio playback is cut off. RAT still operates its VU meter and is receiving data, just no sound comes out. I have an echo-cancelling microphone too, from this kit:

Ive come across this on two XP based machines now.  It dont appear to be a networking issue in that once talk is enable others can be heard fine.
In both cases the machines had different audio cards, and one is single Pc the other a 3 pc system.
